This photograph shows severe late rheumatoid arthritis affecting both hands. Note the anterior dislocation of both metacarpophalangeal joints, with ulnar deviation of the fingers.

It also shows a butonniere or button hole deformity of the right index finger due to rupture of the extensor tendon into the middle phalanx and overaction of the extensors into the distal phalanx.
